Find your EOB

To find your EOB from Midwest Security (UMR) follow these directions:

1 - Go to www.mymidwestsecurity.com
2 - Click on:
 
3 - Enter your user name and password or create a new account if you do not have one.
If you create your account you will need to know our GROUP NUMBER which is 0081318 and your social security number.


4 - Once you access your account click on claims search on the left side of the screen



5- Locate the claim you wish to see and click on the Claim Number

6 - Click to print
